Pediatrics: Pneumothorax



PNEUMOTHORAX

  • Deep Sulcus sign
    • hyperlucent and deep costophrenic sulcus in affected side
  • Double diaphragm sign
    • Air in anterior CPS on lateral film
    • d/t air anteriorly and inferiorly
  • Could be simulated by:
    • Skin folds
    • Bullae
      • concave to chest wall rather than convex (pneumothorax)
  • CT Scan
    • best diagnostic tool
    • linear parenchymal tissue traversing avascular areas

TETRALOGY OF FALLOT

  • Boot-shaped heart
    • Apex lateral and more superior d/t R ventricle enlargement
  • Decreased pulmonary markings
  • Right-sided arch of aorta (also seen in Persistent Truncus Arteriosus)
  • Trachea displaced to the left
  1. High VSD
  2. Aorta overriding VSD
  3. Pulmonary Stenosis
  4. Right Ventricular Hypertrophy

Right to Left Shunt
Aortic enlargement
